Joined up Care Derbyshire

Derbyshire’s Sustainability and Transformation Partnership (STP), known as “Joined up Care Derbyshire (JUCD)”, brings together twelve health and social care organisations across the county with the ambition to provide the best care and services for people and make them as efficient and effective as possible.

8 Place Alliances have been established within the county, to ensure that care and support reflects the needs and views of local communities. Place Alliances will focus on supporting people to stay healthy and independent for longer through a consistent set of work areas which include frailty, falls and care homes.

To support the Place workstream, the Population Knowledge and Intelligence team has developed Place Insight packs incorporating key outcome indicators that underpin moving from a reactive to a proactive care model.
